Please bring along your most recent health check-up report, mammogram film/report, and treadmill report (if available) should you wish to have a comparison made between that and the latest result.

Please bring your valid passport and three photos with a white background (size 4 cm x 6 cm) if you want to combine your Work Permit at the same time incorporate with the annual health check-up.
